ADMISSION
   A wrist tag will be placed with your name upon admission. The tag should not be removed while
     in the hospital because the staff will use this as reference when administering tests and
     medications.
   An admission kit will be issued to you. This consists of a drinking glass, utensils, soap, face      towel, toilet paper, toothbrush, toothpaste and thermometer. All you will have to bring are a few      personal items.
   The hospital supplies all your hospital clothes and linen.

VALUABLES
   The hospital will not be responsible for losses during confinement. We advise you not to bring      any valuables, or you may deposit them for safekeeping with the hospital cashier at no extra cost.
   Every time you leave your room, please be sure to lock it.

MEDICATIONS
For your safety, the hospital requires that all medications prescribed by your doctor are obtained from the hospital pharmacy. We purchase our drugs from reputable manufacturers to assure you of the highest quality.

ROOM TRANSFER
Request for room transfer shall be made anytime of the day through the Admitting Section. Room transfer will be carried out between 8:00 am and 7:00 pm. No transfer will be entertained after these hours.

MEALS
   No trays are served to patient on NPO (Nothing Per Orem)
   Extra trays are charged on a cash basis
   Request for extra tray will be accepted not later than 6:00 am for breakfast, 10:00 am for lunch,      4:00 pm for supper.
   Trays with complete setting are collected an hour after they are brought in.

CHAPLAINCY SERVICES

      Tuesday to Saturday - 5:00 pm
      Anticipated Mass on Sunday - 5:00 pm
      Sunday Mass - 4:30 pm

OTHER RELIGIOUS SERVICES
      Novena of Our Lady of Perpetual Help 5:00 pm Wednesday
      Benediction of the Blessed Sacrament
      every 1st Wednesday and 1st Friday of the month - 4:30 pm

Confession every Friday - by appointment
Guidance & counseling - by appointment
Emergency baptism of infants - anytime, any day (If the chaplain is not available, any of the nurses on duty is authorized to administer baptism.)

STATEMENT OF ACCOUNT
   Statement of accounts are sent to patients / relatives regularly.
   Your daily room rate includes meals and linen.
   Medical / surgical supplies, medicine, diagnostic and therapeutic procedures like x-ray, lab, etc.
   Doctor’s fee (Professional fee) is billed separately from hospital charges.
   It is the policy that patients pay hospital and doctors’ fees in cash

Please settle your accounts at the Billing Section during office hours, as follows:
Monday thru Saturday - 8:00 am to 7:00 pm
Sundays and Holidays - 8:00 pm to 5:00 pm

Only family members of the immediate family may visit and visits are limited at the discretion of nurse supervisor.

Only one watcher for ward and semi-private room patients is allowed. Children below 7 years old are not allowed to visit a patient.

CHECK-OUT AND DISCHARGE
The discharge order is made by your physician. The nurse will send the discharge notice slip down to the Billing Section. The slip will be returned to you with a notation of the time the bills may be taken from the Billing Section. Please wait in your room while your bills are being processed.

After you have settled your accounts, the Cashier will give you clearance slips. Please present these slips to the nurse in charge for her signature. Two copies should be retained by you for presentation to the Admitting Section and lobby guard upon leaving the hospital.

Cut-off time is 12 noon.

Billing for room usage shall be reckoned with check-in time and time of the discharge notice, with the following rules:
        One half day for admission after 12:00 noon
        One half day for discharge before 12:00 noon

FOOD SERVICES
Hospital canteen is open 24 hours.
Convenience Store is open from 8:00 am - 5:00 pm.

EXECUTIVE CHECK-UPS
Check-ups or work-ups are available at package costs. A patient is admitted for a day or two and undergoes a battery of diagnostic tests of your choice. Please call the GHMS (Group of Hospitalization and Medical Services) at local 148 and 483 for more details.

EMERGENCIES
In case of fire or other emergencies, do not use the elevators. The hospital personnel will help you transfer to a safe area.

APPLIANCES
If you wish to bring any appliances, please register it at the Information Section (6 am - 10 pm) / Admitting Section (10 pm - 6 am). All electrical appliances require approval prior to use. Electrical charges will be computed accordingly.

PHILIPPINE HEALTH INSURANCE
Your Philhealth benefits cover hospital benefits up to a certain limit. You can avail of your Philhealth benefits by submitting the duly accomplished PHILHEALTH APPLICATION FORMS and other documenting requirements within 24 hours upon admission to the Philhealth Section of the Billing Office.

AMBULANCE SERVICE
The ambulance is available for conduction upon request. It is fully equipped with all the basic and advanced life support machines and a well-trained crew of nurses, doctors, orderlies and driver. For more details, please call the emergency room at locals 176 and 130.
